Male characteristics are developed through the male hormones testosterone and the more potent dihydrotestosterone. The pharmaceutical company MSD then used this knowledge to fabricate finasteride that puts a brake on this enzyme so that less dihydrotestosterone is made in healthy males that suffer from an enlarged prostate. Once their dihydrotestosterone levels have decreased, their prostate will shrink and symptoms will be relieved. This drug they introduced to the market under the brand name called Proscar. It was then noticed that finasteride triggers hair growth on the scalp and prevents further hair loss. After more research they concluded that a much lower dosage of finasteride was necessary (compared to Proscar) to be effective against male pattern hair loss. As a result, MSD launched another finasteride drug on the market specifically for androgenetic alopecia, called Propecia.

How does Finasteride work?

The enzyme that is responsible for the conversion of testosterone into dihydrotestosterone is present on the scalp. More specifically, in the oil glands of the hair follicles. The dihydrotestosterone causes the hair follicles to shrink which in turn causes hair loss in men. When males take finasteride tablets it will block the enzyme and as a result the amount of dihydrotestosterone that is present in the hair follicles will reduce. The hair follicles can then regenerate themselves; hair loss will be halted and hair regrowth will occur.
